THE ROLE OF PHARMACY TECHNICIAN IN MEDICATION ADHERENCE PROGRAM

ABSTRACT

This study examined the role of pharmacy technicians in medication adherence programs in Anambra State, with a focus on how their knowledge, competencies, and practice contribute to improving patients’ adherence to prescribed therapies. The study was motivated by the increasing challenges of poor medication adherence, which significantly affects treatment outcomes, increases health complications, and raises healthcare costs within the state. A descriptive cross-sectional survey design was adopted, and data were collected from pharmacy technicians working in public and private health facilities across the three senatorial zones of Anambra State. A structured questionnaire titled Pharmacy Technician Medication Adherence Role Scale (PTMARS) was used for data collection.

Data were analysed using descriptive statistics and inferential tests. Findings revealed that pharmacy technicians play a significant role in medication adherence through patient counselling, medication dispensing, follow-up reminders, reinforcement of dosage instructions, detection of adherence barriers, and collaboration with pharmacists and other healthcare professionals. The results also indicated that pharmacy technicians’ involvement improved patients’ understanding of their medications, promoted safe medication use, and reduced non-adherence behaviours. However, the study identified major constraints including inadequate training, limited recognition of technicians’ roles, heavy workload, lack of continuous professional development, and insufficient integration into clinical decision-making processes.

The study concluded that pharmacy technicians are critical contributors to medication adherence programs in Anambra State, and their expanded involvement can significantly enhance patient outcomes. It recommended improved training, structured adherence-support programs, clearer definition of technicians’ roles in healthcare teams, and policies that promote their active participation in pharmaceutical care.

CHAPTER ONE

INTRODUCTION

1.1 Background to the Study

Medication adherence has remained a central challenge in healthcare delivery, particularly in chronic disease management. The World Health Organization (WHO, 2020) defines medication adherence as the extent to which a person’s medication-taking behavior corresponds with the agreed recommendations from a healthcare provider. Poor adherence contributes significantly to treatment failures, increased hospital admissions, and higher healthcare costs globally (Sabate, 2019). In Nigeria, non-adherence is prevalent due to factors such as poor patient counselling, lack of follow-up, limited access to medicines, and socio-economic barriers (Okoro & Onyekwere, 2021).

Pharmacy technicians play a strategic support role within the pharmaceutical care system. They assist pharmacists in dispensing medications, provide basic medication counselling, support inventory management, and help monitor patients’ medication-taking patterns (Adewuyi & Akinleye, 2020). As the demand for pharmaceutical services grows—particularly in underserved areas—pharmacy technicians have become essential contributors to improving medication adherence outcomes (Chukwu et al., 2022). Their interaction with patients positions them to support education, follow-up, and reinforcement of medication instructions.

In Anambra State, the prevalence of chronic diseases such as hypertension, diabetes, and malaria remains a major public health concern (Anambra State Ministry of Health, 2021). Medication adherence programs, which entail strategies like patient education, reminder systems, therapeutic monitoring, and follow-up counselling, are increasingly being adopted across hospitals and community pharmacies. Pharmacy technicians are often deployed to complement the work of pharmacists in implementing these programs (Umeh & Ezeobi, 2022).

The integration of pharmacy technicians into adherence programs enhances service delivery, especially in areas where pharmacists are limited or overburdened (Ogunleye et al., 2020). Their role in patient engagement, monitoring refills, addressing adherence barriers, and facilitating communication between patients and pharmacists has been recognized as vital in improving health outcomes (Jimoh & Bello, 2021). However, empirical evidence on the extent of their involvement and effectiveness in Anambra State remains limited, creating a need for systematic investigation.

1.2 Statement of the Problem

Despite the introduction of medication adherence programs in healthcare facilities, suboptimal adherence remains a persistent challenge in Anambra State. Studies indicate that nearly half of patients on long-term medications fail to adhere to therapy as prescribed, leading to complications such as uncontrolled blood pressure, treatment failures, and avoidable hospitalizations (Okoye & Chukwuma, 2020). Although pharmacy technicians are strategically positioned to support adherence, their actual roles, level of competence, and involvement in adherence programs are not well-documented.

Additionally, many healthcare facilities face shortages of pharmacists, which increases reliance on pharmacy technicians. Without adequate involvement in patient counselling, follow-up, and medication management, adherence programs may not achieve optimal outcomes (Akinola & Yusuf, 2021). The gap in empirical data on how pharmacy technicians contribute to medication adherence programs in Anambra State forms the basis for this study.

1.3 Purpose of the Study

The main purpose of this study is to examine the role of pharmacy technicians in medication adherence programs in Anambra State.

Specifically, the study intends to:

Assess the level of involvement of pharmacy technicians in medication adherence programs.

Determine the roles performed by pharmacy technicians in improving medication adherence.

Examine the challenges faced by pharmacy technicians in the implementation of adherence programs.

Identify strategies to enhance pharmacy technicians’ contributions to medication adherence programs in Anambra State.

1.4 Research Questions

What is the level of involvement of pharmacy technicians in medication adherence programs in Anambra State?

1.5 Research Hypotheses

H₀₁: Pharmacy technicians’ level of involvement has no significant effect on medication adherence programs in Anambra State.

H₀₂: Pharmacy technicians’ roles do not significantly contribute to improving patient medication adherence.

H₀₃: Challenges encountered by pharmacy technicians do not significantly affect the implementation of adherence programs.

1.6 Significance of the Study

This study is significant in several ways. It will enlighten healthcare administrators on the importance of optimizing the role of pharmacy technicians in medication adherence programs. Pharmacists will benefit from understanding how task-sharing with pharmacy technicians can improve patient outcomes. Policy makers such as the Pharmacy Council of Nigeria (PCN) may use the findings to design policies that enhance the training, competency, and deployment of pharmacy technicians.

Furthermore, the study will assist patients by promoting strategies that lead to improved adherence and better treatment outcomes. Finally, the study will serve as a reference material for future researchers and contribute to the literature on pharmaceutical care practice in Nigeria.

1.7 Scope of the Study

The study focuses on the role of pharmacy technicians in medication adherence programs within selected hospitals and community pharmacies in Anambra State. It covers aspects such as their level of involvement, specific functions, challenges faced, and potential strategies for improvement. The study is limited to pharmacy technicians, pharmacists, and selected patients involved in adherence programs.

Medication Adherence: The extent to which patients take medications as prescribed in terms of timing, dosage, and frequency.

Pharmacy Technician: A trained healthcare worker under the supervision of a pharmacist who assists in dispensing, counselling, and medication management.

Pharmaceutical Care: Patient-centered practice aimed at optimizing medication use and improving health outcomes.

INTRODUCTION

1.1 Background to the Study

Medication safety is a critical component of healthcare delivery, as medication errors remain a major cause of preventable patient harm worldwide. A medication error is defined as any preventable event that may cause or lead to inappropriate medication use or patient harm while the medication is in the control of a healthcare professional, patient, or consumer (National Coordinating Council for Medication Error Reporting and Prevention [NCC MERP], 2021). These errors may occur at any stage of the medication-use process, including prescribing, transcribing, dispensing, administration, and monitoring.

Globally, medication errors contribute significantly to morbidity, mortality, and increased healthcare costs. The World Health Organization (WHO) estimates that medication errors cost health systems billions of dollars annually and represent a major public health challenge, particularly in low- and middle-income countries (WHO, 2017). In response, healthcare systems have increasingly emphasized medication error reporting and analysis as essential strategies for improving patient safety and quality of care.

Medication error reporting systems provide structured mechanisms for documenting, analyzing, and learning from errors and near-miss events. Effective reporting enables healthcare organizations to identify error patterns, system weaknesses, and risk factors, thereby facilitating the development of preventive interventions (Reason, 2000; Aronson, 2009). However, underreporting of medication errors remains a persistent challenge, often due to fear of blame, punitive organizational cultures, lack of feedback, and limited awareness of reporting procedures (Uribe et al., 2002; Aljadhey et al., 2013).

Pharmacy technicians play a crucial role in the medication-use process, particularly in medication preparation, dispensing, labeling, and inventory management. Their proximity to dispensing activities places them in a strategic position to identify, prevent, and report medication errors. Despite this important role, most medication safety research has traditionally focused on physicians and pharmacists, with relatively limited attention given to pharmacy technicians’ experiences, perceptions, and reporting behaviors (Szeinbach et al., 2007).

As healthcare systems become more complex and medication regimens increasingly sophisticated, understanding the role of pharmacy technicians in medication error reporting and analysis is essential. Evaluating their knowledge, attitudes, and practices regarding error reporting can provide valuable insights into system-level gaps and inform targeted interventions aimed at strengthening medication safety culture. Therefore, this study focuses on medication error reporting and analysis through a pharmacy technician survey, with the aim of contributing to improved patient safety and quality assurance in healthcare settings.

1.2 Statement of the Problem

Despite the availability of medication error reporting systems in many healthcare institutions, medication errors continue to occur frequently and are often underreported. Underreporting limits the ability of healthcare organizations to accurately assess the magnitude of medication errors and implement effective corrective measures (Kohn et al., 2000). Pharmacy technicians, who are actively involved in medication dispensing processes, may encounter errors or near misses but may not consistently report them.

Factors such as fear of disciplinary action, lack of anonymity, insufficient training, time constraints, and unclear reporting procedures contribute to low reporting rates among healthcare workers, including pharmacy technicians (Hartnell et al., 2012). In some settings, the absence of a non-punitive safety culture further discourages open reporting and learning from errors.

In many developing healthcare systems, limited empirical data exist on pharmacy technicians’ involvement in medication error reporting and their understanding of error analysis processes. This knowledge gap hinders the design of effective policies and training programs tailored to this professional group. Consequently, there is a need to systematically assess medication error reporting practices and analytical awareness among pharmacy technicians. This study addresses this gap by investigating medication error reporting and analysis through a pharmacy technician survey.

1.3 Aim and Objectives of the Study

Despite these disparities, emerging trends such as mobile health applications and blockchain-based drug tracking suggest a growing impact of technology on pharmacy operations.

While technology offers transformative potential for pharmacy technicians in Nigeria, its adoption faces significant barriers, including:

The impact of technology on pharmacy technician roles and responsibilities has been transformative, reshaping traditional tasks and expanding their scope within healthcare settings. Advances in digital systems, automated dispensing, and pharmacy management software have enhanced technicians’ efficiency and accuracy in preparing and dispensing medications. Technology now enables pharmacy technicians to manage inventories more precisely, process insurance claims electronically, and maintain detailed patient records seamlessly, reducing human error and improving compliance with regulatory standards. Additionally, pharmacy technicians engage more actively in customer service and interdisciplinary communication by utilizing digital communication tools to liaise with healthcare providers and insurance companies. Despite technological integration, their core responsibilities—such as verifying prescriptions, labeling medications, and supporting pharmacists—remain vital but increasingly supported by digital solutions. This evolution demands continuous training to adeptly handle sophisticated systems while ensuring patient safety and operational accuracy in diverse pharmaceutical environments.​

1.1 Background to the Study

Technological advancement has rapidly transformed healthcare delivery, with pharmacy practice experiencing significant shifts driven by automation, digitalization, and artificial intelligence (AI). Pharmacy technicians—key support personnel within pharmacy systems—are particularly affected as their roles and responsibilities continue to evolve alongside technological innovations. Globally, hospitals and community pharmacies now integrate technologies such as automated dispensing cabinets (ADCs), computerized provider order entry (CPOE), electronic prescribing (e-prescribing), telepharmacy, robotic dispensing systems, and clinical decision support systems (CDSS), all of which influence workflow and technician responsibilities (Al Ameri et al., 2023; Alotaibi & Aldosari, 2022).

Technology has shifted pharmacy practice from primarily product-focused tasks to more patient-centered responsibilities. Routine dispensing roles are increasingly automated, allowing pharmacy technicians to engage in advanced functions such as medication reconciliation, inventory optimization, data management, and assisting pharmacists with clinical support tasks (Sørensen et al., 2023). These changes not only expand technician competencies but also demand upskilling, continuous training, and adaptation to new work environments (FIP, 2022).

In low- and middle-income countries (LMICs), including Nigeria, the adoption of digital health innovations is accelerating, although implementation remains uneven. Despite infrastructural challenges, hospitals are progressively deploying electronic health records (EHRs), automated dispensing technologies, and digital inventory systems aimed at improving medication safety and workflow efficiency (Okpala et al., 2024). As these technologies evolve, pharmacy technicians in Nigeria face changing expectations in workflow coordination, digital documentation, data interpretation, and operational support roles.

Understanding how technology affects technician roles is essential, considering that pharmacy technicians constitute a large proportion of the pharmaceutical workforce. Their ability to competently use modern technologies directly influences medication safety, efficiency, and quality of care (Koster et al., 2023). Thus, investigating the impact of technology on pharmacy technician roles provides important insights for workforce development, training standards, and policy formulation.

1.2 Statement of the Problem

The rapid integration of digital technologies in pharmacy settings has created both opportunities and challenges for pharmacy technicians. While automation and digital workflows enhance accuracy and reduce medication errors, they also redefine job expectations and require higher technical competencies. Emerging evidence shows that many pharmacy technicians struggle with digital transition due to inadequate training, limited digital literacy, and resistance to workflow changes (Adebayo & Adedeji, 2023).

In Nigeria, the challenges are more pronounced. Although hospitals increasingly adopt digital systems, many technicians report limited exposure to automated dispensing, digital inventory systems, or electronic prescribing platforms (Okpala et al., 2024). This skills gap may result in workflow delays, increased dependency on pharmacists, and reduced efficiency in medication handling. Yet, there is limited empirical research examining how technology reshapes technician responsibilities or the readiness of the Nigerian pharmacy technician workforce to adapt to technological changes.

This study therefore addresses a critical gap by assessing the impact of technology on the roles and responsibilities of pharmacy technicians, particularly focusing on how digital tools, automation, and electronic systems influence their duties, competencies, and job performance.

1.3 Objectives of the Study

The main objective of this study is to examine the impact of technology on pharmacy technician roles and responsibilities.

Specific objectives are to:

Identify the types of technologies currently used by pharmacy technicians.

Examine how technological innovations influence routine and advanced technician responsibilities.

Assess pharmacy technicians’ competency and readiness to adopt new technologies.

Evaluate challenges associated with technology adoption among pharmacy technicians.

Explore strategies for improving technician adaptation and performance in technology-driven pharmacy environments.

For policymakers and regulatory bodies, such as PCN (Pharmacists Council of Nigeria), findings can inform policies on digital health adoption and the modernization of pharmacy technician curricula.

For academics, it expands the literature on digital transformation in pharmaceutical practice, particularly from the perspective of support personnel in LMICs.

1.7 Scope of the Study

The study focuses on examining technological influences on the roles and responsibilities of pharmacy technicians within selected hospital and community pharmacy settings in Nigeria. It examines automation, digital documentation, electronic prescribing, inventory systems, and telepharmacy as the core technological components. The study does not include pharmacists or other healthcare personnel except where their roles intersect with pharmacy technicians.

1.2 Statement of the Problem

INTRODUCTION

Background

Liver damage induced by drugs, toxins, and chemicals is a significant health concern globally. Among the substances implicated in liver injury is paracetamol (acetaminophen), a widely used analgesic and antipyretic agent. Despite its efficacy, overdose or prolonged use of paracetamol can lead to hepatotoxicity, characterized by liver cell necrosis and dysfunction. This hepatotoxic effect has prompted research into potential therapeutic agents that could mitigate or reverse paracetamol-induced liver damage.

Arthocarpus altilis and Its Therapeutic Potential

Arthocarpus altilis, commonly known as breadfruit, belongs to the Moraceae family and is cultivated in tropical regions worldwide. In traditional medicine, various parts of Arthocarpus altilis have been used to treat ailments ranging from hypertension to skin diseases. The seeds of Arthocarpus altilis contain bioactive compounds such as phenolics, flavonoids, and alkaloids, which possess antioxidant and hepatoprotective properties.

Liver damage remains one of the most serious complications of drug toxicity, with paracetamol (acetaminophen) overdose being one of the leading causes of acute liver failure worldwide. Statement of the problem

The findings reveal significant differences in the level of compliance with radiation safety guidelines between private and government facilities. PROTECTIVE EFFECTS OF AQUEOUS EXTRACT OF CITRULLUS LANATUS FRUIT ON REPRODUCTIVE FUNCTIONS AND ANTIOXIDANT ACTIVITIES IN ARSENIC-TREATED MALE WISTAR RATS

ABSTRACT

Arsenic trioxide (As2O3), a known male reproductive toxicant induces it effects majorly through oxidative stress. Citrullus lanatus fruit is widely consumed for its high medicinal values and antioxidant capacities. This study investigated the effects of aqueous extract of Citrullus lanatus fruit (AECL) on reproductive functions and antioxidant activities in arsenic-treated male Wistar rats. Thirty male Wistar rats (150-190 g) were grouped into six and treated as follows; Control, 3 mg/kg As2O3, 100 mg/kg AECL, 200 mg/kg AECL, As2O3+100 mg/kg AECL and As2O3+200 mg/kg AECL. All administration was done orally for thirty days. Caudal sperm, serum hormone levels and testicular antioxidant activities were evaluated. Decreases (p<0.05) in sperm concentration, morphology, viability, and motility were observed in As2O3 group, however, AECL co-administered with As2O3 significantly reversed these effects. Follicle stimulating hormone decreased (p<0.05) in As2O3 group compared with control while significant increase was observed in groups co-treated with AECL and As2O3 relative to As2O3 group. There was an increase (p<0.05) in malondialdehyde level in As2O3 group compared with control while a decrease (p<0.05) was observed in groups co-treated with AECL and As2O3 compared with the As2O3 group. Superoxide dismutase activities decreased (p<0.05) in As2O3 group compared with control but increased (p<0.05) in 200 mg/kg AECL+As2O3 group relative to As2O3 group. Immature spermatids were observed in the seminiferous tubules of As2O3 group, while AECL improved the histology when compared with As2O3 group. The results of this study suggest that aqueous extract of Citrullus lanatus provides protection for sperm cells against arsenic-induced oxidative stress.
